Meghalaya records 574 new HIV cases in three months report by MACS

SHILLONG : Health authorities in Meghalaya have detected 574 new HIV infections in just the first three months of 2026, averaging over six new cases every single day, a figure that has pushed the state’s AIDS control body into launching its most intensive awareness campaign yet.

MACS Project Director Dr KL Iawbor said the numbers point to a clear, ongoing need for sustained behavioural-change work rather than one-off outreach. Data compiled by MACS shows casual sexual contact remains the single biggest driver, responsible for roughly 52 percent of new infections, while regular partners or spouses account for another 32 percent, meaning stable relationships alone now contribute to nearly a third of all fresh cases.

Injecting drug use makes up 9 percent, commercial sex exposure 4 percent, and mother-to-child transmission around 2.5 percent, with zero cases traced to blood products in recent years thanks to tighter screening protocols. Sixteen of the 574 newly detected cases were pregnant women.

“Our effort is to ensure that people come forward for testing without fear and access treatment without facing discrimination,” Iawbor said.

Health Department Commissioner and Secretary Dr Joram Beda framed the numbers against a broader demographic reality: with more than half of Meghalaya’s population under 20, he argued sex education simply cannot remain off-limits any longer. He called the decision to hold the campaign’s launch at Lady Keane College a deliberate one, meant to force open a conversation young people rarely get space for.

“Since time immemorial, sexual health was a taboo topic. Good boys and girls don’t talk about it. Things have changed. Times have changed,” he said, pushing for peer-led education on the grounds that young people often get through to each other far more effectively than teachers or officials ever could.

He was blunt about the scale of the problem too: “This is a very alarming trend, and that is the reason why we are here, that we need to talk, we need to start talking about this.”

Health Minister Wailadmiki Shylla, who formally launched the two-month Statewide Intensified IEC Campaign 3.0 alongside International Youth Day observances, said tackling HIV can’t be left to MACS or the Health Department alone. He called for an end to discrimination against people living with the virus and pushed government departments, schools, community groups, healthcare workers, parents, and young people themselves to treat this as shared responsibility.

He specifically encouraged students and college Red Ribbon Clubs to get actively involved, describing them as natural channels for spreading accurate information among peers.

The rollout itself is genuinely large in scale. Over the next two months, the campaign will run across all 12 districts through roughly 1,026 village-level meetings, close to 3,000 school outreach sessions, and 85 college sessions spanning around 110 institutions, alongside 30 dance performances, flash mobs at 30 marketplaces, outreach to about 15,000 households, 12 special camps, and 10 pop-up awareness drives.

District-level implementation will run through Deputy Commissioners and Block Development Officers, while door-to-door efforts will lean on Dorbar Shnongs and local youth clubs.

Beda also linked the state’s HIV challenge to rising substance abuse, warning that drug use could compound transmission risk, and urged young people specifically to stay cautious.

He pushed back against the fear that often surrounds a diagnosis, pointing to how far treatment has come. “Nobody dies of HIV these days, by the way. It’s just like any other disease,” he said, while still stressing that the virus’s ability to spread quickly makes early testing and treatment essential.

The 574 new cases add to an already serious longer-term picture. MACS data compiled in November 2025 put Meghalaya’s total confirmed HIV/AIDS caseload above 10,000, including nearly 500 children, with the state’s adult prevalence rate running at roughly double the national average.

Officials have noted that Meghalaya’s epidemic looks different from several other Northeastern states, where injecting drug use tends to dominate, here, sexual transmission accounts for the overwhelming share of cases, a pattern health.

Officials also said the crisis can no longer be treated as confined to any narrowly defined “high-risk” group.
